         1. Przedmiotem zamówienia jest dostawa płynów dializacyjnych i substytucyjnych oraz płynów do perfuzji i przechowywania narządów dla UCK w ilości, asortymencie oraz wymaganiach określonych w załączniku nr 3 do SIWZ.

         2. Zamawiający dopuszcza składanie ofert częściowych. Pod pojęciem oferty częściowej rozumie się pojedyncze części ustalone w załączniku nr 3 do SIWZ. Wykonawca może złożyć ofertę na jedną lub kilka części.

         3. W zakresie części 1 Zamawiający dopuszcza składanie ofert na pojedyncze pozycje ustalone w załączniku nr 3 do SIWZ. Wykonawca może złożyć ofertę na jedną lub kilka pozycji.

         4. Zamawiający nie wyraża zgody na dokonywanie zmiany załącznika nr 3 do SIWZ – formularz asortymentowo-cenowy pod rygorem odrzucenia.

                           6. Wszelkie nieuregulowane w niniejszym SIWZ czynności, uprawnienia, obowiązki Wykonawców

                           I Zamawiającego, których ustawa nie nakazała zawierać Zamawiającemu w SIWZ, a które mogą przyczynić się do właściwego przebiegu postępowania, reguluje ustawa PZP.

                           7. Zamawiający przewiduje dokonanie zmian umowy w toku jej realizacji w przypadku zaistnienia okoliczności, o których mowa w art. 144. Dz.U. z 2017 poz.1579 ze zmianami
