         Przedmiotem zamówienia jest wymiana w aparacie GAMMAMED plus iX, numer seryjny A043 źródeł promieniotwórczych Ir-192, polegająca na dostawie nowych źródeł Ir-192, utylizacji zużytych źródeł poza granicami Polski oraz dokonaniu czynności wymiany, przy czym:

         Zamawiający zamawia realizację 12 (dwunastu) wymian źródeł promieniowania Ir-192 w ciągu 36 miesięcy.

         Wymiana w aparacie źródeł promieniotwórczych Ir-192 obejmuje następujące czynności:

         1) dostawę źródła do miejsca wymiany w miejscu zainstalowania aparatu,

         2) wymianę źródła,

         3) odbiór zużytego źródła oraz transport i utylizacja poza granicami Polski; Odbiór zużytego źródła następować powinien jednocześnie z instalacją nowego źródła,

         4) dostarczenie wymaganych dokumentów źródła, jak również sporządzenie protokołów przekazania do eksploatacji, protokołu odbioru zużytego źródła i protokołu przeglądu.
