         — w zakresie Pakietu Nr 1 – zestawu do laparoskopii 1 kpl.,

         — w zakresie Pakietu Nr 2 – diatermii chirurgicznej 1 sztuka,

         — w zakresie Pakietu Nr 3 – diatermii chirurgicznej 1 sztuka.

         Których wymagane parametry techniczne określono w Załącznikach Nr 6.1 – 6.3 do SIWZ, ich montaż i uruchomienie, a także świadczenie gwarantowanych usług serwisowych w okresie na jaki udzielona jest gwarancja na oferowany przedmiot zamówienia oraz przeszkolenie pracowników w zakresie obsługi u Zamawiającego.

               Odwołanie wnosi się do Prezesa Krajowej Izby Odwoławczej zgodnie z art.180 ustawy Prawo zamówień publicznych, w terminie:

               — 10 dni od dnia przesłania informacji o czynności Zamawiającego stanowiącej podstawę jego wniesienia – jeżeli zostały przesłane w sposób określony w art. 180 ust. 5 zdanie drugie, albo w terminie 15 dni – jeżeli zostały przesłane winny sposób,

               — 10 dni od dnia publikacji ogłoszenia w Dzienniku Urzędowym Unii Europejskiej lub zamieszczenia SIWZ na stronie internetowej wobec treści ogłoszenia i SIWZ,

               — 10 dni od dnia, w którym powzięto lub przy zachowaniu należytej staranności można było powziąć wiadomość o okolicznościach stanowiących podstawę jego wniesienia,

               — 30 dni od dnia publikacji w Dzienniku Urzędowym Unii Europejskiej ogłoszenia o udzieleniu zamówienia,jeżeli Zamawiający mimo takiego obowiązku nie przesłał Wykonawcy zawiadomienia o wyborze oferty najkorzystniejszej,

               — 6 miesięcy od dnia zawarcia umowy, jeżeli Zamawiający nie opublikował w Dzienniku Urzędowym Unii Europejskiej ogłoszenia o udzieleniu zamówienia, jeżeli Zamawiający mimo takiego obowiązku nie przesłał Wykonawcy zawiadomienia o wyborze oferty najkorzystniejszej.
