        1. Przedmiotem zamówienia jest dostawa wyrobów medycznych stosowanych w ortopedii dla UCK

        W asortymencie, szacunkowej ilości oraz wymaganiach określonych w załączniku nr 3 do SIWZ.

        2. Asortyment wymieniony w pkt. 1, musi spełniać wymagania określone w załączniku nr 3 do SIWZ. Niespełnienie chociażby jednego z wymagań określonych w załączniku nr 3 do SIWZ spowoduje odrzucenie oferty.

        3. Zamawiający nie wyraża zgody na dokonywanie zmiany Załączniku nr 3 do SIWZ – formularz asortymentowo – cenowy pod rygorem odrzucenia.

        4. Zamawiający wymaga, aby zaoferowany asortyment spełniał wymagania dotyczące wyrobów medycznych zawarte w Ustawie z dnia 20.5.2010 roku o wyrobach medycznych (Dz.U. nr 107, poz. 679 z późn. zm.) oraz w wydanych na podstawie tejże ustawy, aktach wykonawczych.

        5. Dostawy będą następować sukcesywnie, w ilości i asortymencie, zgodnie z zamówieniami częściowymi Zamawiającego w terminie do 6 dni od daty otrzymania zamówienia.
